DHF remediation for scaling medtechs

Design histories decay as teams grow. Remediation brings yours back to a state you can defend, and the right system stops the gaps reopening.

DHF remediation is the work of bringing an incomplete or inconsistent design history file back to a state you can defend: every design input traced to outputs, verification and validation, every record reviewed and approved, and one current version of each document. For a scaling medtech, it is also the moment to stop the gaps reopening.

Why gaps appear as teams scale

Early on, the design history lives in a few people's heads and a shared drive. As the team grows, documents multiply across Word, SharePoint and spreadsheets, people move on, and the links between requirements, risks and tests stop being maintained. Nobody decides to let the file decay. It happens one shortcut at a time.

Step one: inventory

List every document the design history should contain, against your design and development plan, and find the current approved version of each. Note where there is no approved version, or several candidates.

Step two: gap analysis

Check traceability end to end: user needs to design inputs, inputs to outputs, outputs to verification, needs to validation, and hazards to risk controls. Rank each gap by the risk it carries, not by how easy it is to fix.

Step three: remediate honestly

Rebuild missing traces, re-review documents that were never approved, and repeat verification that tested an earlier design. Date every remediation record when it is created and label it as remediation. Backdating a record to make the history look tidy turns a documentation gap into an integrity problem.

Step four: stop it reopening

Remediation fails if the same habits continue. Move the remediated documents into a controlled system where traceability, review and versions are maintained as people work, so the design history stays current without a second project.

How Traceable helps

Traceable imports your existing DOCX and XLSX files, so remediation starts from what you have. Requirements, risks and tests are linked with TraceIDs, broken traces surface as you work, and every document goes through review and e-signature before it is published. Common questions

Can we backdate records during remediation?

No. Date every record when it is created and label remediation work as remediation. A clear, honest remediation record is defensible, and a backdated one is not.

How long does DHF remediation take?

It depends on the size of the gaps and the risk they carry. Starting from a complete inventory and a gap analysis ranked by risk lets you fix what matters first and show progress early.

Do we need to stop development to remediate?

No. Remediate in order of risk alongside development, and move each document into a controlled system as you touch it, so new work does not add new gaps.
