Columns to include
A workable requirements traceability matrix does not need to be complicated. These columns cover most regulated projects.

A spreadsheet matrix works until requirements start changing. Common questions
What columns should a requirements traceability matrix have?
At minimum: a unique requirement ID, the requirement text, the source or parent it traces up to, the design output that implements it, the verification method, the test case that verifies it, and a coverage status. The table above lists each with a short description.
How do I keep the matrix from going out of date?
